| Eligibility | To qualify for ODSP income support, you must:

| Application | Applicants for the Income Support program must fill out a Disability Determination Package which can be obtained at local ODSP or Ontario Works offices after a meeting with the Income Support Specialist/Caseworker to determine financial eligibility. Forms must be filled out by the applicant and his/her doctor. Applications must be filled out and returned no later than 90 days after receiving the package.
