| Description (Service) | Direct referral and early intervention program for individuals living with Alzheimer disease and related dementias. Provides easy access to comprehensive and coordinated services by reaching out to persons with dementia and their caregivers as early as possible in the disease process and throughout the continuum of the disease. |
