Description
Description (Service) | This person-centered program works with people to determine what is personally important for them to achieve. Team members help individuals to identify meaningful goals and effective ways for each person to reach those goals. Staff works alongside clients, enabling them to exercise their rights and to enjoy a meaningful life in their community while striving to achieve their full potential. Works to promote health, housing, purpose, and community. The program uses a holistic, strengths-based approach to support individuals towards living a full, independent and satisfying life. Individuals must be engaged with their worker towards identifying and achieving goals aimed at promoting recovery. This is a time limited service that is focused on supporting individuals to live as independently as possible. |

Eligibility | For individuals who are 16 years or older and live with a serious mental illness and/or substance use disorder. Must have a willingness and a commitment to work on personal recovery goals. |
Application | Referrals accepted from hospital, inpatient units, family doctor, community organizations and health services. Call the Central Referral and Intake Team. |
