Description (Service) |
Supports for older adults, and adults with disabilities to help them remain independent in their own homes for as long as possible. Comprehensive community, health and in-home services are provided by nurses, personal support workers and volunteers. Programs include: - Caregiver support circle - a free program for adult caregivers at home caring for another adult (family member or friend) in the Grey-Bruce area, see separate listing for details.
- Community Nursing - General nursing care including visiting. Includes assessment, treatment and education of wound and ostomy care, education for people with diabetes, pain and symptom management, palliative care, TB skin testing clinics. Various locations throughout Grey and Bruce, contact for details.
- Falls Prevention and Group Exercise Classes for Seniors - Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions. Aims to improve strength and balance to prevent injury and falls. See separate listing for details.
- Home At Last Program - Assists older adults returning home from a hospital stay. Provides transportation from hospital and meal on arrival at home. Can pickup required medication (doesn't pay for medication). Also provides check-in call the following day.
- Supportive Care Program - Hospice Visiting Service - Companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-limiting or life-threatening illness. Bereavement support for anyone who has experienced the death of someone close. Services can be provided at home, or in hospitals and long-term care homes. Palliative care education.
- Assisted Living - Care for older adults with chronic illness or cognitive impairment who require assistance with independent living.
- Cool-Aid - free program for adults living with disabilities and seniors 55 plus, ensures medical information efficiently accessed in emergency situation. Joint program by VON Grey Bruce with Grey Bruce Paramedic Services
