In person at applicable ServiceOntario Counters * see website for locations currently offering this service * bring original identity documents to prove your legal name, date of birth and signature and marriage certificate if applying under married name Card will be mailed in 4 to 6 weeks. If individual has no identity documents they can obtain a Declaration from a Guarantor form |
