Description

Description (Service) Provides community support and advocacy to individuals who experience significant mental health concerns, using an individual case management approach following recovery principles.

Services are provided by nurses and social workers and include:
  • complete assessment
  • coordination of care and service
  • life skills training
  • individual counselling and support
  • supportive group treatment for communication, coping, crisis planning, stress management and life skills
  • health education about mental disorders and medication use
  • therapeutic social and recreational planning with participation in peer lead community activities
  • referral and advocacy to other community agencies or partners

Areas Served Bruce County ; Grey County
Eligibility Ages: 16 year(s) and up Persons residing in Grey or Bruce county who are experiencing significant mental health concerns

Application Self-referral, medical referral or referral from community agency. Call the Community Mental Health Team for inquiries or referrals.
