Description (Service)

Services include:

  • Campbell House: 10 bed palliative care residence with beds allocated to end of life support as well as short stay, supportive palliative respite and pain and symptom management
  • In-home visiting volunteer support: Offers trained volunteers to provide emotional support to individuals living with life-threatening illness, and their caregiver(s) and to the bereaved via the phone or in person.
  • Professional individual and group bereavement support and counselling is offered by a team of social workers and psychotherapists via the phone, in person or on zoom.
  • Drop in grief groups to support those in our community who are grieving and to help foster connection to create a support network for individuals.

The Hospice Georgian Triangle Foundation (registered charity #83108 5089 RR0001) accepts donations to fund the operations and services provided by Hospice Georgian Triangle.

Hours Campbell House (for patients and families): Mon-Sun 24 hours
Hospice Georgian Triangle Programs and Administration Office: Mon-Fri 9 am-5 pm
The Hospice Georgian Triangle Foundation Office: Mon-Fri 9 am-5 pm
Areas Served Blue Mountains ; Clearview (Simcoe County) ; Collingwood ; Wasaga Beach

Eligibility Ages: 18 year(s) and up Individuals who are eligible for admission to Campbell House are those suffering from a life-limiting illness with a prognosis of three months or less and are not seeking any curative treatment. Individuals must have involvement with Ontario Health atHome.

Support and Programming is available to anyone who is living with a life-limiting illness, their caregiver(s) or anyone who is grieving a loss in the service area.

Application Referrals for Campbell House are made through the Ontario Health atHome and your Home Care Coordinator

Referrals for caregiver(s), bereaved individuals seeking grief support/counselling or community members living with a life-limiting illness and grieving children for support and programming can be made via self referrals or by other health care and support organizations.
